If you have had a Gail Borden Library card within the last three years, DO NOT COMPLETE THIS FORM. Please visit the registration desk at the main library or the customer service desk at the Rakow branch if you need a replacement for a lost card.

When you or your guardian sign our in-house form, you will receive your new library card.

You will have 30 days to bring two forms of I.D. ( click for Accepted ID List ) with your current street address information to the Library. If you have not come in during that time period, your registration will be deleted from the system.
