Passport Services

The Gail Borden Public Library District has joined with the U.S. Department of State to provide passport services. The Main Library is an official Passport Acceptance Facility, accepting DS-11 applications on the second floor of the Main Library.

 

Please make sure you allow enough time for the U.S. Department of State to process your passport application before you are scheduled to travel. If you would like to know how long it will take to receive your passport, you can review detailed information by visiting the U.S. Department of State's website.

Applications are processed by appointment only.  To make an appointment, please call 847-429-4693.  Please note, each application may take 30 minutes to process.

Passport Services is closed on Fridays.

Passport Application Requirements
To apply for a passport, you will need to provide the following:

1. Completed Passport Application Form DS-11
Applications may be filled out online at www.travel.state.gov and then printed single-sided. The Library also provides paper applications.

2. Proof of U.S. Citizenship (one of the following)

  • Original birth certificate or certified copy issued by the city, county, or state, listing both parent's names
  • Previous U.S. Passport
  • Certificate of Naturalization
  • Certificate of Citizenship
  • Consular Report of Birth Abroad or Certification of Birth

3.  Proof of Parental Relationship (for minors)

  • Original birth certificate or certified copy issued by the city, county, or state, listing parent’s names
  • Original adoption papers
  • Original legal guardianship papers

4. Proof of Identity (original of one of the following)

  • Valid driver's license (or State ID)
  • Certificate of Naturalization
  • Prior U.S. Passport
  • Government employee ID
  • Official Military ID card

5. Current Passport Photo
A color photo of yourself (2 inches by 2 inches) taken within the last 6 months is also required. The Library provides photo services at a cost of $12. You may also have your photo taken at CVS, Walgreens, Sam's Club, and other locations.

6. Processing Fees
For detailed fee information, visit the U.S. Department of State's website. All payments made to the U.S. Department of State must be made by check or money order.  A separate check is needed for each application. The execution fee to the Library (Acceptance Agency) must be made as a separate payment.
The Library can accept credit or debit cards for payments of library fees only. For specific fee information, please contact the Library.

Passport Application for Minors

To apply for a passport for a minor under the age of 16, the child and both parents must be present.  If one parent is unable to attend, a notarized DS-3053 Statement of Consent form must be submitted along with a copy of the front and back of the non-applying parent's ID.  A minor age 16 or 17 must have proof of parental consent.  Minors under the age of 16 will also need an original certified birth certificate even if they have had a previous passport.

Adult Passport Renewal

The Library does not accept mail-in DS-82 renewal applications. To process your renewal, please visit the U.S. Department of State's website.

If you were under 16 when your passport was issued, you cannot renew. If your passport is expired and was issued more than 15 years ago, you cannot renew. If your name has changed, you cannot renew without legal documentation of your name change. For all these circumstances, please follow the above instructions for a DS-11 passport application.
